Abstract
Growth discordancy in twin gestation is a well known phenomenon.
Because neonatal data support birth weight discrepancy as the most significant factor in the prediction of neonatal morbidity and mortality in discordant twins, sonographic monitoring of estimated fetal weight percentage difference may prove to
be
a
considerable benefit in management.
The present study was undertaken in 25 twin pregnancies of discordancy of more than 15% at birth from Jan. 1st 1985 to Dec. 31th 1991, in order to evaluate the perinatal outcomes associated with birth order of smaller infants and value of
ultrasonography by intrapair difference in abdominal circumference in detecting discordant fetal growth.
@ES The results were as follows;
@EN 1. The larger the discordancy, the greater the likelihood that the smaller twin would be second, although no statistically significant difference.
2. Discordant second twins were smaller than discordant-first twins(P<0.05), which probably reflected the larger number of growth-retarded and lower apgar score infants. This was probably an important cause for higher perinatal death rate of
these
twins. But no significant difference was between discordant-first and cordant twins associated with perinatal outcomes.
3. Cutoff value of 20 mm or more for abdominal circumference difference was suggested to be significantly useful in predicting discordant fetal growth as a screening test (P<0.05).
